Ongoing Assessment Tool

We're starting something new this year as a tool to monitor the students throughout the year. Each student will have a file folder where they can decorate the outside to make it personalized and inside will look like the picture above. The right side is based off of what The Sisters have in their "pensieve", but modified so that it is a little simpler. On the left are things that we will continue to monitor the students' progress in (letter/sounds, number recognition, counting, etc.). The hundreds chart is for when listening to the students count - circle how far they counted to. I like it because it lets the students visually see how far they counted to. 

I'm excited about this file folder idea because it lets anyone coming in to work with that student know at a glance how he/she is doing. There is a section called "all about me" where you can write what you have learned about that child - likes, enjoys, etc. so that another teacher working with that child will automatically be able to make a connection. I really wanted to make something that will not be seen as "just another thing to fill out" but something that you can actually use in small groups (math, reading, writing) and have all that data in one place. I plan on punching 3 holes in them and keeping it all in a binder. The fonts are from Kevin and Amanda - Pea Hollee and Smiley Monster. I also used a basic print one that was on my school computer - abcprint.

    Thank you SO much for sharing this! I can absolutely use this, especially during parent teacher conferences! I plan on using a different color for each nine weeks! LOVE it! :)

    How did you do that? Read my mind like you just did?! My idea was far too complicated; this is simple and easy to use.

    Question: so each child would have their folder and you would keep the folders in a class binder?

    @ Nicole - thank you! answer - each child will decorate their folder but I will hold onto them in a binder for me to use during small groups/assessment time. I do plan on letting them see it while I use it so we can celebrate successes together!

    Have you read the book The Leader In Me by Sean Covey? The schools in that book use data notebooks that the kids fill in themselves. I had wanted to do something like that this year but was running out of time. Your file folder idea is perfect for Kindergarten to help them learn to track themselves. Thanks for sharing!

    @Mrs. Geier- I found a digital scrapbooking kit that I liked and designed it using adobe photoshop. I learned a lot from kevin and amanda's site (blog tutorials):

    love your idea but I would use it better if the letters were out of order, or in the order that we teach them. Also the numbers out of order too.

    Heather,
    Are you trying to print the word document? If so, it might be all jumbled up if you don't have the same fonts. I'd recommend changing the fonts to something you already have. If you like it just the way it is, you should print the pdf version. Hope that helps!

    I love this for parent teacher conferences! I tried to download the papers, but the formatting is all weird when I try. Is there a way to fix that? I'd love to use this!
